Our multidisciplinary health professionals and case managers take a collaborative approach to mental health care by linking with their patients to work together and consult within an environment appropriate to the person's needs.The Pathways to Community Living Initiative (PCLI) was established as part of the NSW Governments commitment to assisting long stay patients with severe and enduring mental illness to live in the community. The clinical target group for this position is older people with complex mental illness and ageing issues who are either long stay patients in a mental health facility or who in future could be long stay patients. Consumers in this group typically have severe, enduring and/or complex mental health problems with associated poor functioning, chronic physical illness, and also severe Behavioural and Psychological Symptoms of Dementia (BPSD).The Specialist Mental Health Service for Older People (SMHSOP) Clinical Nurse Consultant - Grade 2 (CNC2) will be responsible for the provision of expert clinical advice to consumers, carers and other health care professionals on complex mental health care needs within the Mental Health service. As a member of the Community SMHSOP Team, the Clinical Nurse Consultant provides specialist nursing services covering the spectrum of assessment, treatment, and management of older people with severe and persistent mental illness residing in Residential Aged Care Facilities (RACF) and those relocating from long term mental health inpatient care. The Clinical Nurse Consultant will be expected to provide supervision, education and mentorship as part of this role, building capacity within both the community SMHSOP team, and within RACFs.We invite you to take on your next career milestone with us.   • Minimum five years full time equivalent post qualification experience, with at least three years full time equivalent experience in mental health nursing and post graduate qualifications in mental health or equivalent experience deemed appropriate by the Health Service
  • 3.Demonstrated effective communication and inter-personal skills across a variety of mediums and an understanding of the practice development framework
  • Proven ability to implement changes and develop direction in clinical practice by the utilisation of evidence based research
  • Demonstrated skills in strategic and clinical service planning including an understanding of organisational behaviour, change management and cultural change processes
  • Demonstrated skills in clinical teaching, competency in the use of contemporary information systems and experience in the development of specialty education programs
  • 7Understanding of the recovery philosophy and trauma informed care in Mental Health with the ability to implement these in clinical practice
  • A current NSW drivers licence, including P1 or above, and willingness to use same as required for the position
