In every aspect of our work, we are committed to helping whānau be the best they can be.He kōrero mō te tūranga | The RoleAt our Transitional Service our purpose is to support tangata whaiora to integrate back into the community within a two-year time frame. As a Support Worker, you'll provide wrap-around support to Tangata Whaiora between the age group 18-25 years, in a hands-on capacity in a service that has a whanau orientated framework.This is a varied role, and your day-to-day duties will depend on the needs of the individual Tangata Whaiora. You'll coach, mentor, and work with Tangata Whaiora to gain real skills by encouraging their participation with day programme activities, support them with individual goal plans and encourage wider whanau and community connections.This is a permanent full-time position working 40 hours per week, 24/7 rostered shifts. This position will be based in Wellington.Your duties will include but are not limited to:
  • Establish a rapport with tangata whaiora and their whanau.
  • You'll support with medication management and helping tangata whaiora be as autonomous as possible.
  • Provide day to day support in alignment with tangata whaiora goals and support plans including risk management plans and Safety Plans.
  • Attend and contribute to team meetings, bringing ideas and updating others on their activities.
  • Ensure all incidents are documented appropriately.
He kōrero mōu | About YouYou'll contribute to maintaining a safe working environment and create safe, calm, and nurturing spaces for Tangata Whaiora. You'll encourage whānau to engage in therapy, meaningful programs, and educational/vocational activities.We are looking for someone who is positive, flexible, vibrant, confident, and compassionate to work in our service. You'll naturally be a problem solver, quick thinker and enjoy connecting with the community in line with the reintegration plan. You'll use your non-judgmental attitude and belief to encourage clients to change and grow.Ideally you will have the following:
  • Experience working in a similar role.
  • A Level 4 (or higher) qualification in Mental Health and Addiction or be working towards one.
  • Demonstrated commitment to recovery values.
  • Advanced communication and interpersonal skills.
  • Eligibility to work in New Zealand.
  • It's essential that you have a Full New Zealand driving licence.
  • Demonstrated commitment to the Treaty of Waitangi and culturally safe practice.
