-Opt in below to receive text message updates on CDHS recruiting events!Please note: This recruitment may be used to fill multiple vacanciesThis position will receive a short-staffing temporary pay differential of 3.17% from July 1st 2023 - June 30th 2024.About Grand Junction Regional CenterThis work unit exists to provide quality health care, personal hygiene maintenance, achievement of personal outcomes and a therapeutic milieu in a home-like environment for up to 12 individuals with developmental disabilities and mental illness. About the Position:
The aim of the position is to give support and services to people in the areas of health, safety, activities of daily living and also providing opportunities for person to achieve their desired personal outcomes as described in their Individual Plans. Duties in this position include:Directly provide and assist with daily living care for residents:
  • provide care and instruction in daily living activities for persons receiving services, such as bathing, eating and personal hygiene, to ensure personal outcomes are met as determined by their Individual Plans
  • with supervision and /or delegation, successfully apply knowledge in administering treatments and medications as prescribed for Waiver services only
  • medication administration procedures including, but not limited to: administering, monitoring self-administration; PRN medications; Standards, precautions and safe practice; Preparing and /or altering medication for administration; Controlled substances, counting, administering, documenting; Proper documentation of medication administration; Medication errors, documenting, reporting; Medication Storage; Medication Disposal; Medication reminder boxes and day/trip packs, filling and administration
  • administer dietary orders as prescribed. This would include preparing meals to prescribed diet consistencies as specified in the dietary planning menu.
  • respond to emergency situations and assists medical staff with medical support and services
Individualized Support Services:
  • follow and implement assigned individualized supports and services for residents. Programs include behavior, social, recreation, work and specialized therapies; residential and day program areas and community settings
  • document and/or communicate per protocols/procedures all required and pertinent information regarding residents for whom this position is responsible for at any given time
Day Programming:
  • provide/assist in the implementation of learning, support or experience programs
  • report any issues/concerns
Home care & meetings:
  • refine the physical condition of the living site by addressing safety concerns, making approved purchases, decorating living areas and performing housekeeping duties
  • report any safety/hazard issues
  • attend assigned meetings and in-services as scheduled by the supervisor to fulfill the mission/vision of the agency
Transportation:
  • transport individuals receiving services to/from appointments, scheduled events, and emergency transports.
  • transport individuals to programming and on outings
Other Duties as Assigned:
  • other Duties as assigned
  • may be required to work other locations and/or shifts as needed to address agency needs
  • must be annually certified for the skilled implementation of Safety Care
Special Qualifications:
  • must have a current LPT license in good standing or be QMAP-trained
Minimum Qualifications:
  • high school diploma or General Education Diploma (GED)

  • Shift Work - GJRC is a 24/7 facility; therefore, person in this position must be available to work any shift, any day, and/or any area to meet agency needs.

CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults - Mental Health Institutes, Regional Centers, Veterans Community Living Centers) * TB test is required, Requires Influenza and COVID vaccine as required by facility
