Connect with your Talent Advisor for additional specifics.ResponsibilitiesThe Vice President of Population Health is an executive leadership position reporting to the CEO and is a key leader in the development of hospital and physician practice strategy and tactical plans. This position is responsible for working with executive, physician, nursing and operations leaders to ensure alignment with the Maryland Total Cost of Care Model. This position will lead the development of sustainable initiatives intended to empower and engage patients and the community in activities that improve health outcomes and reduce the social determinants of health. Areas of responsibility include care management across the full continuum, utilization management, value based post acute services, community health, chronic disease management, behavioral health and mobile primary care.Duties:
- Establish a comprehensive population health strategy and program that supports the organization's mission, vision, values, and financial goals.
- Participate in organizational operations, strategy and planning as an executive leader
- Lead a large department of hospital and community based programs with competing priorities
- Translate vision into strategic and tactical execution of population health based programs.
- Think innovatively and strategically about differentiators and critical needs to improve total cost of care performance
- Work with leadership to engage, enable, and empower providers through incentives, tools, data systems, and clinical resources to accomplish population health goals, improve health outcomes and organizational objectives.
- Communicate with executive leadership and Ascension corporate leadership to champion population health initiatives that align with financial incentives in Maryland.
- Ensure a successful population health strategy through working in collaboration with key stakeholders.
- Maintain professional affiliations, and current knowledge of applicable laws, regulations, and statutes regarding quality, and remain current with the latest trends in healthcare.
- Develop/maintain a management structure which ensures the establishment of a unified, effective population health strategy.
- Establish partnerships with the physicians, administrative staff and employees, supporting their ability to implement the “Quintuple Aim”.
- Associate's degree/Bachelor's degree with 7 years of applicable cumulative job specific experience required, with 4 of those years being in leadership/management.
- Experience developing, leading and executing population health strategies across a system or managed care operation.
- Demonstrated experience leading value based care programs and/or experience working within the Maryland Total Cost of Care environment.
- Minimum of 5 years of experience directing health programs (clinical or prevention).
