- At least three to five years of strong acute care case management experience
- Bachelors in Nursing required; MSW required
- Masters Degree in Nursing, Social Work, Healthcare Management, or comparable field preferred
- For Social Work, LCSW preferred
- Case Management Certification ACM, CCM preferred; to be completed within 12 months of hire preferred
- New Jersey Registered Nurse OR Licensed Social Worker
- Travel Required within Northern Region Daily to Weekly
- Opportunity for a hybrid schedule after a pre-determined onboarding period
- Manages a caseload of psychosocially, legally, and/or medically complex patients referred via campus case management departmental leadership
- Engages assigned patients in any setting, including acute, post-acute, ambulatory settings, and home based upon the needs and agreed upon goals of the complex case team
- Provides case management and discharge planning services with a goal of mitigating delays and achieving goal length of stay
- Participates and attends hospital committees/boards/meetings which pertain to area of expertise, i.e. LOS meetings, complex discharge meetings, other board and committee meetings as appropriate to specific assignments, etc.
- Provides timely, consistent updates to leadership and clinicians involved in care of patients
- Provides feedback and education as needed to care team, including clinicians and unit/service level case management staff
- Conducts interviews with patients and families with cultural humility
- Accurately assesses problem situations
- Uses purposeful strategy to intervene in problem situations
- Educates patients/families and facilitates referrals to community resources; serves as a liaison among patients, medical staff, the hospital at large and outside community agencies
- Communicates clearly and concisely both verbally and in writing
- Prepares and submits complete and accurate reports as needed by leadership
- Documents patient-level activities in patients medical records
- Provides accurate and timely documentation according to departmental guidelines, including compliance related needs Patient choice, 2nd IM, etc.
- Assists as needed in a variety of complex care management tasks, including but not limited to:
- Identification of unknown patients admitted to the hospital
- Preparing for and/or appearing at guardianship proceedings
- Liaising with foreign consulates and embassies
- Collaborating with governmental organizations such as APS, DDD, DCP&P, etc.
- Consulting medical ethics and palliative care
- Facilitating communication among specialty consultants
- Navigating the intersection of medical illness and serious and persistent mental illness
- Addressing concerns for futile care
- Leverages resources for uninsured/underinsured patients
- Demonstrates professional development on the job as evidenced by:
- Stays current with field of practice through journals, articles, and other pertinent literature
- Maintains an accurate and up to date database resources and referrals
- Show initiative in application of current scope of practice knowledge
- Develops complex discharge plans; analyzes and solves specific problems
