- Implement the case management approach to support families in accessing public benefits and services.
- Work closely with families to assess their unique needs and circumstances.
- Develop and manage individualized service plans for each family.
- Assist families in enrolling in various public benefit programs, including adult education, employment assistance, childcare programs, housing and rental assistance, and healthcare programs.
- Provide guidance and support throughout the application process.
- Build rapport and maintain positive relationships with families.
- Offer emotional support and encouragement to families facing challenges.
- Serve as a resource and advocate for families to access the services they need.
- Maintain accurate and confidential records of client interactions and services provided.
- Collaborate with the Family Resource Director to compile and report on program data as needed.
- Conduct outreach activities to identify and engage eligible families in need of support.
- Collaborate with community organizations, government agencies, and service providers to connect families to resources.
- Facilitate workshops and training sessions for families to enhance their knowledge of available resources and self-sufficiency.
- Keep up-to-date on changes in public benefit programs and share information with families.
- Collaborate closely with the Family Resource Director and other team members to ensure the seamless delivery of services.
- Participate in team meetings and contribute to program planning and improvement efforts.
- Maintain confidentiality of all information being reviewed, processed, and filed.
- Maintain compliant caseload, based on funder mandates.
- Perform any additional responsibilities established by the Family Resource Director.

- Bachelor's degree in social work, human services, or a related field preferred.
- Relevant experience in social services, case management, or a related field.
- Knowledge of public benefit programs and community resources.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with diverse stakeholders.
- Commitment to the mission and values of the YMCA of South Florida and The Children's Trust.
- Bilingual, English and Spanish preferred.
- Valid Driver’s License.
- Adult/ Child CPR & First Aid certified.
