OVERVIEW:To provide direct Speech Therapy services to participants through the Article 16 clinics, including, providing physician ordered speech therapy services, assessment/evaluations, planning, developing, and implementing an individualized patient therapy plan. On a referral basis, provide indirect services to program and residential participants, consultation, training, written plans of care and any other require duties as assigned, function as an Interdisciplinary Treatment Team member and ensure compliance with all applicable program standards. Serve as trainer to new hires and/or supervisory staff.RESPONSIBILITIES:Article 16 Clinic Services:
  • Provide physician ordered speech therapy services.
  • Ensure physician orders are renewed, per OPWDD Article 16 requirements.
  • Responsible for completing, in a timely manner, all clinical documentation including, but not limited to, initial assessment/evaluations, daily notes, renewal orders, progress reports, and discharge summaries.
  • Identify patient short/long term goals in terms of functional outcomes, reflect progress or lack of goal achievement in all charting, and confer with physician and/or interdisciplinary team regarding patient response to therapy.
  • Complete communication and swallowing evaluations and develops appropriate communication and feeding programs.
  • Help patients work towards accomplishing their treatment goals by utilizing therapeutic, assistive devices.
  • Attend scheduled interdisciplinary staff meetings.
  • Supervise ST students/new grads as assigned.
Indirect Clinical Services:
  • Complete observations of diet consistency, as requested. Recommend equipment and assistive devices as appropriate for dinning needs.
  • Update/develop Individualized Plans of Care or Conditions based upon the individual’s needs.
  • As requested, if an individual is not receiving direct services through the clinic, evaluate and instruct patient and caregivers in safe dining practices and positioning, or if appropriate, make a referral to receive direct therapy through the Article 16 clinics.
  • Provide training/supervision to direct care staff regarding implementation of treatment plans developed by the ST as needed.
  • Provide consultation to other team members as needed for ST needs.
  • Instructing, enhancing, and developing staff training programs for employees.
  • Provide staff training/in-servicing as needed or requested, which may include: Choking Prevention Training and condition based staff training
  • Attend scheduled interdisciplinary staff meetings administrative meetings, case reviews, and ITT’s when requested.
  • Complete all trainings as assigned.
  • Maintains professional knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.
  • Participate in committees and program development, as indicated.
  • Assists Associate Director/Manager with budgeting for ST Department needs. Request needed ST supplies and equipment.
  • Perform all other duties relevant to the position as assigned by supervisor.
  • Regular and reliable attendance is required.
ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:
  • Possess or is eligible for a Certificate of Clinical Competence (CCC)
  • Licensed and currently registered as a Speech-Language Pathologist by the New York State Department of Education, or eligible for licensure by virtue of holding a Certificate of Clinical Competence (CCC) issued by the American Speech-Language-Hearing Association (ASHA)
  • Must be at least 18 years of age
  • Reliable transportation (NYS Drivers’ Licensed required)
  • Ability to work independently and motivate others
  • Ability to communicate effectively, both orally and in writing
  • Ability to lift at least 50 lbs. dead weight
  • Maintain all required certifications/training by State regulations and CDS Life Transitions policy
  • Experience working with individuals with developmental disabilities preferred
