Day ShiftsJob Summary:
We are currently seeking compassionate and skilled Registered Nurses (RNs) to join our Home Health team on a temporary basis. As a Temp RN in Home Health, you will provide comprehensive nursing care to patients in their homes, helping them to recover from illness or injury, manage chronic conditions, and maintain their independence. The ideal candidate will have previous experience in home health nursing, strong clinical skills, and a commitment to delivering patient-centered care.Responsibilities:
  • Conduct thorough assessments of patients' health status, including physical, psychological, and environmental factors, to develop individualized care plans.
  • Provide skilled nursing care, including wound care, medication management, IV therapy, and patient education, in accordance with physician orders and established protocols.
  • Perform home visits to assess patients' condition, monitor treatment progress, and provide ongoing support and education to patients and families.
  • Coordinate care with interdisciplinary team members, including physicians, therapists, social workers, and home health aides, to ensure comprehensive and coordinated services for patients.
  • Monitor patients' vital signs, symptoms, and response to treatment, documenting observations and communicating findings to the healthcare team.
  • Educate patients and families about their health conditions, treatment options, and self-care techniques, empowering them to participate in their own care and make informed decisions.
  • Document all patient assessments, interventions, and outcomes accurately and in compliance with regulatory requirements, using electronic medical records systems.
  • Communicate effectively with patients, families, and healthcare team members to ensure continuity of care and promote patient safety and satisfaction.
  • Adhere to infection control protocols, safety guidelines, and organizational policies to maintain a safe and healthy environment for patients and staff.
Qualifications:
  • Valid Registered Nurse (RN) license in the state of practice.
  • Minimum of two years of clinical nursing experience, with experience in home health, community health, or hospice nursing preferred.
  • Strong assessment, communication, and interpersonal skills.
  • Ability to work independently and collaboratively in a dynamic and changing environment.
  • Commitment to providing compassionate and patient-centered care to patients in their homes.
