- Conduct initial intake with client and family/guardian.
- Create, track & organize physical files for assigned workload.
- Ensure the timely completion of all required documentation (ie: intake, session notes, surveys, dismissals etc.) in accordance with SWK policy.
- Enter documentation in internal database system ETO and funder database PEIRS after each session.
- Run reports in ETO to track clients and services.
- Keep consistent communication with families in assigned client list.
- Plan and create lesson plans to facilitate weekly group sessions in the office or assigned community sites using the designated curriculum.
- Ensure curriculum is delivered professionally and effectively to meet desired learning objectives.
- Maintain proper sign-in sheets of all clients attending group sessions and activities.
- Facilitate and/or co-facilitate assigned group sessions and recreation sessions.
- Ensure clients participate or are offered all components or services offered at the program.
- Deliver and collect required surveys for assigned clients and enter collected data.
- Provide advocacy, referrals and linkages to program participants and their families.
- Maintain positive working relationships with community partners to allow for collaborations and provide program services at various locations.
- Collaborate with school staff and community agencies for service delivery and outreach opportunities.
- Work within the community to promote program offerings; attend/organize outreach events as assigned.
- Recruit and retain participants in programming in coordination with other program staff.
- Meet monthly and annual target numbers as assigned.
- Regular and punctual attendance.
- Able to react to change productively and handle other essential tasks as assigned in order to meet needs of the program.
- Attend and participate in all required professional trainings and workshops.
- Maintain open and consistent communication with team leader and Program Director/ Program Coordinator.

- Maintain workspace clean and organized; assist with office cleaning tasks as assigned.
- On occasion provide transportation for clients to attend program events & activities.
- Work evening and weekend hours as needed.
- A bachelor’s degree in social service or a related field with at least two (2) years of experience working with at risk youth and their families; may include part time, volunteer or internship experience.
- Experience working with youth ages 6-17.
- Works well independently and as an effective team member.
- Computer literate with working knowledge of Microsoft Windows (Microsoft Word, Excel, PowerPoint), variety of online database systems and applications.
- Possess good verbal and writing skills.
- Bilingual in Spanish and English
- Clear background check from appropriate entity
- Must possess a valid state driver’s license, registration and maintain state mandated car insurance.
- Well organized, meticulous, and thorough in work habits.
