- Establish, collaborate, and maintain relationships with applicable Coordination Entities, Emergency Service Providers, outpatient clinics, and other providers/entities to share information and ensure that the needs of the person are fully met.
- Provide crisis intervention assistance to people served through direct intervention or warm handoffs and referrals as appropriate.
- Oversee the development and implementation of Safety Plans and Crisis Prevention and Response Plans.
- Build relationships with Persons and develop shared understanding of Persons’ goals and the steps necessary to achieve goals.
- Participate in On-Call rotation and critical incident review process
