- Identifies, manages, and treats specific skin conditions.
- Completes a thorough and accurate wound assessment upon notification of wound, then weekly and as needed. Documents all assessments in the medical record.
- Observes for any changes in residents' condition and report to physician and resident/representative, and documents accordingly.
- Provides direct care skills (wound care, catheterization, tube feedings, etc.) in accordance with current guidelines.
- Assists with resident assessments as scheduled and as needed.
- Assists with direct resident care as needed.
- Participates in the admission, discharge, and transfer of residents as required.
- Plans, develops, organizes, implements, and directs restorative services for residents who require those services in the facility.
- Confers with interdisciplinary team in development of comprehensive care plan.
- Participates in QAPI activities as needed or assigned.
- Assess each resident for problems and needs through conversation, observation, and a physical assessment and documents accurately.
- Establishes a culture of compliance by adhering to facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines.
- Is able to give constructive feedback and discuss resident care issues that conflict with quality resident care.
- Assists with open shifts as needed.
- Other duties as assigned.
- Proficient oral and written communication skills.
- Strong multi-tasking skills with the ease of changing directions upon request as needed.
- Sound analytical and problem-solving abilities.
- Ability to work with others and possess the communication and interpersonal relationship skills needed for a leadership position.
- Assessment skills to identify and recognize resident physical and mental needs,
- Comply with HIPPA and other federal, state and local regulations as well as maintain the highest degree of confidentiality in client and staff matters
- Maintain skills through training provided as well as self-study.
- Graduate of a state approved nursing program.
- Currently licensed as an LPN/RN in the State of Wyoming.
- Completion of the SV Health/SVCC orientation program.
- At least one year's experience in long term care is preferred.
