THE ROLEAs a Case Worker - AOD Outreach Program, you will play a pivotal role in developing holistic treatment plans targeted to individual needs of clients who are experiencing substance abuse or dependence. In this dynamic & rewarding role you will travel around Armidale & the New England region, to deliver vital Outreach AOD support services to smaller rural communities. You will be joining a supportive and inclusive team where you can contribute meaningfully to the community and make a real difference to people's lives.RESPONSIBILITIES
Conduct assessments and develop evidence-based treatment plans and therapeutic interventions for individual clients.
Work in partnership with broader health services e.g. mental health services and other primary health clinicians, Aboriginal medical services.
Maintain accurate client records, case notes and other data as required.
Provide ad hoc support as needed, e.g. psychosocial support, conflict resolution etc.
Engage in case review processes with the team, and individually with the Team Leader.
YOU WILL NEED
Relevant qualification/s, e.g. Certificate IV or Diploma in Alcohol and Other Drugs or Community Services field, or extensive experience in the AOD/Community services field.
Demonstrated experience in managing a case load and case planning, including with persons with complex needs.
Demonstrated ability to work therapeutically with clients (e.g. provide therapeutic interventions).
Knowledge of, and an ability to access, a range of relevant community resources.
Drivers Licence & willingness to travel around the Armidale & New England Region & provide outreach support 3 days per week.
