We have an urgent need for a FT PT to service our locations in Bradenton Beach to St Petersburg Florida.
This is a salaried position but must be willing to travel during the workday to see patients at home in Manatee and Pasco County.Onsite only-Direct HireSalary, Benefits and BonusWe are offering retention bonuses at 6months and 1 year. But must be there for the time period specified.Reports to: Director of Nursing JobSummary: Professional member of patient's treatment team who evaluates, assesses, and delivers services according to a written plan of treatmentestablished by a physician. Provides supervision for all services rendered by a physical therapy assistant. Evaluations are performed bypeers.Job Responsibilities: Duties of the registered physical therapist include, but may not be limited to:1. Assist physician in evaluating patient's level of physical functioning and need for physical therapy services2. Discuss evaluation with physician to help establish a plan oftreatment to assist patient to meet maximum rehabilitation potential3. Carry out prescribed treatments; and/or supervise PTA in carrying out PT programs4. Establish a home program to be followed by the patient/significant other5. Advises and consults with the family and other agency personnel and instruct patient/significant other in home programs6. Observe and report to physician pertinent changes in the patient'slevel of functioning. Communicate changes to other team members involvedin the patient's care.7. Prepares clinical progress notes and maintain appropriate documentation of all services as per agency policy8. Conduct self in professional manner at all times and in all situations9. Provide agency with all required evidence, including copy of license/registration, to allow agency to verify experience10. Accept only those assignments for which he/she is qualified11. Participates in in-service programs as per agency requirement12. Comply with all state, federal, local and agency rules, regulations, policies, and proceduresQualifications:Minimum: Bachelor's degree from an accredited PT program approved by the council on Medical Education of the American Medical AssociationLicensed/registered in the State of the HHA as a physical therapist Minimum of two years home health experience as a physical therapist andone year supervisory experience.
