If you have a passion for working with people, this is a position for you.DESCRIPTION OF WORKAn Aging Care Manager 2 works with older adults from initial contact with individuals, their families and professionals, through ongoing care management responsibilities. Responsibilities include specialized duties (Protective Services, Care Transitions, Person-Center Counseling, etc.) You will have the opportunity to maintain a caseload of consumers and be involved in necessary areas of assistance. You may regularly coordinate services with physicians, community agencies, financial institutions, etc. Work includes managing multiple tasks and close attention to detail, which includes completion of paperwork, work assignments and meeting deadlines.Work Schedule and Additional Information:
  • Full-time employment.
  • Work hours are 8:00 am to 4:30 pm, Monday - Friday, with 60-minute lunch.
  • On-call hours when required.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONSMinimum Experience and Training Requirements:
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.
