By developing authentic relationships with residents and family members, they help residents lead lives of enrichment and ensure their wellness needs are met.Example of dutiesA DAY IN THE LIFE:
We are committed to providing preventative, restorative, and person-centered care to all residents at Rockingham County. We want our residents to feel safe, comfortable, and supported in all aspects of their lives to include not only physical, but also emotional, social, spiritual, environmental, and intellectual well-being. Our PCP's are a vital part of our dynamic team! The PCP actively engages with residents in a friendly, caring, and warm manner by initiating conversation and activities while supporting residents in doing as much for themselves as possible and further, promoting positive leisure time. When needed, they provide hands-on care that may include assisting with meals, laundry, showering, vital signs, and more. Our PCP's are continually educated on the best documentation practices, and are also responsible for observing/reporting any changes or concerns of physical, psychological, and social changes to the Wellness Coordinator or Wellness Manager. Additionally, they are responsible for following all established safety and infection control policies/procedures and will also intervene/report unsafe situations.Typical qualificationsREQUIRED EDUCATION AND EXPERIENCE:
  • High School Diploma or GED; and
  • Two years related experience in a Health Care setting and/or training; or
  • An equivalent combination of education, work and experience will be considered.
  • CPR/Basic Life Support (BLS) Certified.
  • National Caregiver Certification Course (NCCC) welcomed, not required.
  • Licensed Nursing Assistant (LNA) License welcomed, not required.
