- Community Health Choice (HMO D-SNP), a Medicare Advantage Dual Special Needs plan for people with both Medicare and Medicaid that combines Medicare Part A and Part B benefits, Medicare Part D prescription drug coverage, and Medicaid benefits with additional health benefits like dental, vision, transportation, and more.
- Essential Functions
- Educate, advise, and guide members on nutrition plans that help them maintain a healthy lifestyle and improve health outcomes. Identify health goals and dietary restrictions.
- Marginal Functions
- Collaborating with interdisciplinary team to improve health outcomes
- Collaboration on workflow development and creation of care models
- Education/Specialized Training/Licensure: Bachelor degree in dietetics, foods and nutrition or a related area/ Registered Dietician required.
- Bachelor degree in dietetics, foods and nutrition or a related area preferred.
- Work Experience (Years and Area): 3 years required.
- 5 years preferred.
- Software Proficiencies: Proficient in MS Office (Word, Excel, Outlook)
- Experience with Government populations required.
- Texas Medicaid Experience, Medicare and MarketPlace experience preferred.
