- Takes initiative to create and implement programs to increase revenue and client population
- Responsible for the care coordination of clients
- Establishes and maintains relationships with referral sources
- Monitor care plans to ensure client needs continue to be met
- Coordinate client weekly billing and transmittal
- Performs supervisory visits
- Responsible for scheduling staffing needs for all convenience services
- Conducts caregiver orientation/training
- Coordination of client and caregiver schedules, approval of time slips and entering corrections/changes in scheduling software
- Caregiver coaching and counseling as required
- Addresses client and/or caregiver concerns and complaints in a timely manner. Performs investigation as needed.
- Ensures timely submission of all required caregiver paperwork
- Readily available via cell phone for on-call
- Supervises staff in Director’s absence
- Maintains a high degree of confidentiality at all times due to access to sensitive information
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements
- Abides by all regulations, policies, procedures and standards
- Performs other duties as assigned
- Maintains positive internal and external customer service relationships
- Maintains open lines of communication
- Plans and organizes work effectively and ensures its completion
- Meets all productivity requirements

- Actively participates in continuous quality improvement
- Represents the organization professionally at all times
- Associate’s degree in related field
- 5 years health care experience preferred
- Certified Nurse’s Aide in good standing with the state is preferred
- Strong communication skills and interpersonal skills
- Ability to work independently
- Valid driver’s license and proof of insurance is required
