Elderwood is currently seeking a Assisted Living Regional Director of Clinical Services (RN) for our Assisted Living portfolio!Multi-site experience in Assisted Living Facilities required.This position requires on-site presence in Western New York and Rochester facilities; some travel to Albany and Vestal, NY.Regional Director of Clinical Services (RN) - Position Overview:
  • The AL-RDCS will provide clinical oversight, in conjunction with the CNO, to a portfolio of nursing facilities in the states in which the company operates. The individual will have and express a genuine interest in caring for elderly patients and have experience in clinical operations in long term and post-acute space. This is position is of an executive nature, comprised of complex leadership and administrative components including, participating in company activities, recruitment, hiring, training and the development of policies and procedures.

ResponsibilitiesAssisted Living Regional Director of Clinical Services (RN) * Acts as a nurse mentor and advisor to regional nurse consultants, facility leadership and assist in Director of Nursing orientation, by being available for consultation, keeping current with medical and nursing care processes, keeping abreast of changes in health care delivery systems and methods and providing information to facility leadership as indicated or requested.
  • Travels as required to various facilities, conferences, and meeting which may include overnight stays.
  • Performs hours of work based on need and the amount of time necessary to perform the job responsibilities including on-call and emergencies.
  • Acts as the clinical lead and adviser during the transition of new facilities.
  • Conducts, prepares, and participates in routine Director of Nursing meetings.
  • Provides onsite support with annual state and federal survey inspections, as needed.
  • Knowledgeable of and assists with implementing state and federal regulations and the company’s policies and procedures.
  • Assists with developing and reviewing plans of correction following a state or federal survey in cooperation with facility leadership.
  • Assists, as needed or requested, in development of adequate staffing patterns and ratios in
cooperation with facility leadership. * Timely notifies company leadership of events which pose potential compliance and/or litigation risk.
  • Assists facility leadership with managing, investigating, or providing follow-up education related to such events. Conduct root cause analysis of negative outcomes, as necessary.
  • Monitors for trends or quality improvement opportunities, by routinely reviewing audits, nurse consultant visit reports, mock survey reports and investigations.
  • Serves as the clinical lead and performsmock survey audits. Performs onsite review of resident care and services as requested, to ensure compliance with state and federal regulations and company policies and procedures. Provides a verbal and/or written report of findings and recommendations for follow-up to the Administrator, Director of Nursing, appropriate company leadership.
  • Maintains effective communication with facility leadership by reporting all observations while onsite to the Administrator and Director of Nursing. Conducts entrance and exit conferences with the Administrator and/or Director of Nursing or designees during each onsite visit.
  • Communicates routinely and effectively with the CNO and RDO regarding facility clinical performance, site visit schedules, survey activity, events or other clinically relevant information.
  • Assists with nurse recruitment and retention efforts as needed/requested. Participates in interview and selection process for community nursing leadership positions as needed/requested.
  • Observes and emphasizes safety awareness and accident prevention to facility staff.Activity C F O N
  • Reviews quality measures and quality data (CMS 5 Star, CASPER, QM/QI, Quintile Reports, etc.), assists facility leadership in development of performance improvement programs (PIPs), action plans and reviews outcomes in relation to established benchmarks. Conducts data review for statistical comparison and trend analysis of information as requested.
  • Notifies company leadership of events which pose potential compliance and/or litigation risk timely.
  • Assists facility leadership in managing, investigating, or providing follow-up education related to such events. Conducts root cause analysis of negative outcomes, as necessary.
  • Monitors for trends or quality improvement opportunities, by routinely reviewing audits, nurse consultant visit reports, mock survey reports and investigations.
  • Provides oversight and guidance to regional nurse consultants in developing and conducting ongoing training programs for nursing and other staff in response to QAPI programs to promote quality care and survey readiness at all times.
  • Facilitates, attends and actively participates in QAPI Meetings and Risk Meetings at the facility level, as requested. Provides oversight and feedback to facility leadership and/or regional nurse consultants on quality measures and quality data, and QAPI, development of organizational performance improvement programs, action plans, PIPs, and outcomes in relation to established benchmarks.
  • Creates, revises and updates policies and procedures accordance with changes in standards ofpractice, state, and federal regulations.
  • Participates in healthcare associations and professional organizations.
  • Collaborates and coordinates with the company’s business partners including contracted consultants, vendors and other healthcare providers and partners.
QualificationsAssisted Living Regional Director of Clinical Services (RN):
  • Degree in Nursing from accredited educational institution, BSN preferred
  • Current registration in good standing as a Registered Nurse (RN) instates assigned facilities are located or ability and willingness to obtain
  • Demonstrated evidence and experience as Director of Nursing
  • Operational experience in Long Term Care and Skilled Nursing (RDCS SNF); Assisted Living (RCDS ALF)
  • Experience in oversight of a multi-facility organization
  • Knowledge of state and federal regulations, specifically:
  • RDCS SNF – LTC
  • RDCS ALF – AH, EHP, ALR, SNALR, EALR, ALP
  • Must have reliable transportation; current, valid driver license
  • Proficiency in various computer programs, applications, electronic communications, electronic medical records
  • Regional Nurse Consultant experience preferred
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
