Clinical Social Worker

• Assists in patient’s adjustment to their illness/condition and adherence to the treatment plan, as well as ensuring that they are aware of, and have access to, appropriate community resources. Includes patients/caregivers in the involvement or formulation and implementation of the patient's plan of care, and interdisciplinary collaboration will be employed to ensure the greatest continuity of care.

• Provides social work services to patients and their families.
• Education and Counseling.
• Maintain resource materials.
• Documentation.
• Collaboration/Conference.

• Minimum Education:

• Required:

• Master’s Degree in Social Work
• Preferred:

• None

• Required:

• Three years social work experience, preferably with older adult population; experience with community agencies helpful.
• Preferred:

• Three years social work experience, preferably in Home Health.

• Required:

• New Jersey Licensure for Social Work, LSW or LCSW.
• Valid NJ driver’s license.
• Preferred:

• None

• Required:

• Strong clinical assessment skills and the knowledge and ability to respond to routine social work related inquiries.
• Knowledge of local community resources.
• Preferred:
