The Patient Outreach Coordinator is responsible for the coordination of Fee for service and PPO patient referrals, to include prior authorization submission, tracking, patient scheduling, and closing the loop on all referrals as well as the coordination of treatment for hospital and/or ancillary services. The Patient Outreach coordinator serves as a liaison between patients, physicians, office staff, and health plans to ensure a coordinated and efficient referral process. The Patient Outreach Coordinator provides a seamless consistent process of referral for Fee for service and PPO patients across all providers/specialties. Ensures a white-glove experience for Fee for service and PPO patients accessing care in our system. Required ExperienceRequired:
  • High school diploma or equivalent
  • 1 year of patient referral experience in a medical office or hospital setting
  • Previous experience with ICD-9 & CPT coding systems
  • Knowledge of medical terminology
  • Bilingual (Spanish) required
Preferred:
  • Basic knowledge of frequently ordered diagnostic tests
  • Graduation certificate from an accredited Medical Assistant program.
