Works under the supervision of Care Manager (RN or PT) to provide personal care, comfort measures and environmental services for patients (including those needing special precautions) and performs specified treatments. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. The individual must demonstrate knowledge of the principles of growth and development over the life span and provide the care needed as described according to the departmental policy and procedure. Promotes and maintains safe work environment for staff as well as our customers, incorporating national patient safety initiatives. Must have reliable transportation to perform assigned home care/hospice visits. The Home Health/hospice Aide may be required to work weekends and holidays as census and staffing require.What you will do
  • Provides personal care, comfort measures, environmental services, and specified treatments for a specified number of patients as determined by department standards.
  • Assists with client's personal hygiene needs, ambulation, exercise, and with medications that are ordinarily self-administered (according to state regulations). Reports any changes in the client's condition and /or needs to the assigned supervising Registered Nurse. Provides nutritional support as needed or directed. Provides other supportive tasks as assigned and/or specified by the case manager. Hospice aides performs duties consistent with Hospice Aide training and permitted to be performed under State Law. Ensures availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes. Recognizes emergency situations and responds competently.
  • Ensures availability of resources (equipment, instruments, supplies) necessary to ensure positive patient outcomes. Maintains clean, safe and orderly work/patient environment by cleaning work stations/areas, equipment, stocking supplies, and placing equipment in proper storage areas.
  • Assists and participates in training and support of all team members, volunteers and students.
QualificationsEducation
  • Program Graduate, Nursing Assistant program approved by VA Board of Nursing (Required)
Experience
  • Prior Nurse Aide work experience in a home care setting (Preferred)
Licenses and Certifications
  • Certified Nurse Aid (CNA) - Virginia Department of Health Professions (VDHP) (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
  • First Aid - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 30 Days(Required)
  • Valid Drivers License Required
