- Complete assessments, exit services plans and provides support with travel coordination and arrangements to help asylum seekers transition out of the city's supportive housing and resettlement services.
- Contact family, relatives, or others to coordinate travel.
- Provide resettlement services outside of the city's supportive housing.
- Complete referrals to community-based programs and organizations
- Completes case notes, data, and information in the NY Health + Hospitals case management system.
- Coordination with interpretation and translation services
- Complete referrals to immigration legal services
- Complete other tasks as necessary to facilitate delivery of necessary services to clients.
- Performs other job-related duties as assigned.
- Basic understanding of immigration laws, regulations, procedures, and benefits/entitlements.
- Excellent verbal and written communication skills and ability to learn quickly and problem-solve under pressure and within prescribed timelines.
- Excellent verbal, written and communication skills and the ability to work collaboratively with families, collaborating agencies, and community professionals.
- Ability to organize, multitask and prioritize duties and responsibilities efficiently.
- Proficient in utilizing Microsoft Excel, Power Point, Word, and Outlook.
- Flexibility required due to potential shift changes based on operational needs.
