- Act as program liaison to the Executive Office of elder Affairs, State agency Area Offices (DDS, MCB), and other Department personnel as delineated by Program Director.
- Act as liaison with other agencies involved in providing services to the people we support, such as Day Programs.
- Ensure that the program meets and complies with all applicable Medicaid regulations; oversee compliance of providers with mandated trainings.
- Process participant referrals and the coordination of the admission process, as identified in State Agencies’ regulations regarding "placement services”.
- Participate in recruitment, selection, and evaluation of Adult Family Care families. Interview families and facilitate the matching and transition of participants into homes.
- Develop positive relationships with participants, families and monitor/assess placements minimally on a regular basis.
- Train and supervise families in all facets of their responsibilities.
- Support the development and growth of all participants served in the program.
- Work with the AFC Registered Nurse as part of the multidisciplinary team.
- Identify, monitor and complete certain section of the MDS/HC assessment, as well as a psychosocial assessment.
- Ensure and maintain that all program record keeping, documentation and reviews are accurate and up to date; daily log, monthly case manager notes, home inspections, site safety plans, health record, psychosocial assessment etc.. Develop and complete part of the participant’s Plan of Care and annual MDS assessment.
- Bachelor’s Degree in related field with four years’ experience; or Master’s Degree and two years of experience.
- Ability to communicate effectively verbally and in writing.
- Intermediate computer skills and knowledge of Microsoft Office products.
- Ability to develop and maintain satisfactory working relationships with employees and providers.
- High energy level, superior interpersonal skills and ability to function in a team atmosphere.
- Must hold a valid driver's license and have access to an operational and insured vehicle.
- Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations.
