- Assess home health patients to identify the physical, psychiatric, psychosocial, and environmental health needs
- Provide comprehensive psychiatric care to patients in their homes, delivering high-quality nursing services and support to individuals with mental health needs
- Upon admission will assess and document: mental status, including psychological and behavioral status; sensory and motor function; cranial nerve function; and language function
- collaborate with multidisciplinary teams, assess patients' mental health status, develop care plans, and administer treatments to ensure the well-being and stability of patients within their home environments.
- Provide skilled nursing care and treatment interventions to patients with psychiatric disorders, including medication management, therapeutic interventions, and counseling support.
- Supervises, teaches, and provides clinical direction to other nursing personnel, assigning home health aides to specific patients. Supervises LPNs/LVNs providing services to patients
- Maintain accurate and up-to-date patient records, including assessment data, care plans, progress notes, and discharge summaries, ensuring compliance with regulatory standards and best practices.

- Current state license as a Registered Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC)
- Applicable education and experience requirements for the options below:
- Master’s degree with a specialty in psychiatric or mental health nursing AND nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services; OR
- Bachelor’s degree in nursing AND 1 year of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services; OR
- Diploma or associate degree in nursing AND 2 years of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
- Current Driver’s License
- Proof of current CPR certification, if required to perform in-home visits
