Conducts and documents detailed psychosocial assessments/evaluations to interpret social, environmental, and medical needs of the patient/family. Meets the needs of transplant applicants and donors and/or level ii evaluations to determine mental illness/mental retardation discharge needs.
Reassesses, monitors, and documents discharge planning needs and interventions. Ensures that medical coverage is obtained by securing the appropriate documentation and materials. Evaluates the effectiveness of the plan of care and recommends alternative plans as needed.
Provides or supervises plan of care interventions to include resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate.
Serves as field instructor for social work students and provides supervision to LMSW's for certification as assigned; assists with orientation and mentoring of employees.
Participates in performance improvement initiatives to include data collection and analysis, development and participation in continuing education programs, development and editing of policies and procedures, and serving on committees; prepares and maintains applicable reports and records.
Conducts follow-up phone calls to patients/families to ensure proper healthcare coverage; serves as a patient advocate; interacts with local, state, and federal officials.
