If so, we have an exciting opportunity for you!Child and Family Guidance Center is seeking a Youth and Family Wraparound Case Manager to play a key role in providing intensive case management services to families of children with severe emotional disturbances. In this role, you’ll employ the National Wraparound Implementation Center (NWIC) wraparound model to engage with families, identify strengths, and develop highly individualized Plans of Care.What we offer

What you’ll do
  • Manage a caseload of 10-13 children/families, providing 60 hours of services monthly
  • Facilitate team meetings to develop comprehensive, family-driven Plans of Care
  • Provide weekly case management, monitor progress, and modify plans as needed
  • Assist families in accessing community resources and support
  • Maintain accurate documentation and uphold professional relationships with families
  • Prepare families for program transitions and provide home visits as needed
What you bring
  • Bachelor's degree in rehab counseling, psychology, social work, or a related field
  • Minimum of 2 years related experience and/or mental health training preferred
  • Previous experience in case management or related field preferred
  • Proficient writing skills for professional communication and documentation
  • Proficient in Microsoft Outlook, Excel, Word, and Medisoft billing and scheduling system or similar product
