Job Description Are you looking for a great opportunity to work day shifts on the weekend? TeamHealth is looking for a nurse practitioner to come work for our post-acute care team! Responsibilities:
  • Drive high-quality, safe, and cost-effective patient care in post-acute care communities
  • Collaborate with interdisciplinary teams and follow CMS guidelines
  • Work closely with hospice, palliative care, and home care agencies for safe patient transitions
  • Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines
Qualifications:
  • Experience in internal medicine (IM), family medicine (FM), or geriatrics preferred
  • Previous experience in post-acute care, traditional medicine, outpatient, urgent care, or hospitalist settings is a plus
