- Provide clinical nutrition expertise through nutritional assessments and documentation.
- Coordinate with Interdisciplinary Team and the Corporate RD to address every resident’s care needs and to ensure all standards are exceeded
- Implementation of plans of correction
- Conduct nutritional assessments for new admits, significant weight changes, pressure injuries, dialysis, and calculate TPN and enteral nutrition for residents in the health center.
- Assess potential nutritional risks and implement appropriate medical nutrition therapy interventions.
- Provide nutritional counseling and education for residents and families.
- Collaborate with the dining department to ensure nutrition plans are executed effectively.
- Responsible for resident charting, audits, and clinical documentation requirements
- Review past surveys and audits and ensure all deficient areas are now deficiency free.
- Review QA practices within the dining room, residents’ rooms, and kitchen meet all local, state, and federal regulations.
- Communicate issues, concerns, and opportunities for improvement to the Executive Director, Dining Director, Executive Chef, Director of Nursing, and Corporate RD.
- Monitor and validate nutritional systems for the community.
- Provide training to hourly team members and dining leadership at the community as indicated by Quality Indicators, changes in regulations, and audit results.
- Conduct facility-wide presurvey audits in preparation for the long-term care, CMS survey.
- Conduct kitchen, dining room, and pantry presurvey audits in preparation for the Assisted Living survey.
- Stays informed of federal and state regulations for long-term care, dietetic registration/licensure, and food safety.
- Stay up to date with industry trends, new products, current research, and new programs in medical nutrition therapy and foodservice, and provide ideas and recommendations to the Corporate Registered Dietitian, community Dining department, and other HumanGood Registered Dietitians.
- Bachelor's degree in dietetics, nutrition, or a related field required. Masters preferred.
- 2+ years of experience in a long-term care environment or geriatric nutrition.
- Registered Dietitian with the Commission on Dietetic Registration of the Academy of Dietetics and Nutrition.
