In collaboration with other clinical disciplines, this role integrates these evidence-based practices to facilitate optimal coordination of care, patient, family, and clinical staff communication and education, and optimal transition/discharge planning following CMS (Medicare's) Condition of Participation. The RN Case Manager serves as an expert in coordinating the patient care trajectory through the continuum of care and that the patient is at the right level of care that is appropriate.The Case Manager must demonstrate knowledge of growth and development to coordinate patient care appropriate to the age of patients served on his/her assigned clinical area. The employee must be able to successfully complete job-specific orientation, Case Management specific competencies and meet/exceed standards as identified in the criteria-based performance appraisal.Day to day, you would be assessing the needs of the patient and family to coordinate, monitor, evaluate, and advocate for multiple services to meet the specific patient's complex needs. You would collaborate with a multidisciplinary team regarding the patient's needs based on the assessment.Required: Certifications/Licenses in IN or compact licensure states:Active Registered Nurse (RN)Case Management Certification is preferred.Education & Experience:Graduate from an accredited school of professional nursing is required.BSN preferred or educational plan in place.Recent clinical experience in specific areas required for specialty clinical groups or clusters, preferred for others.A minimum of three (3) years of relevant clinical experience is required.Case Management experience preferredWhy Choose Deaconess?Deaconess Health System is the largest employer in the tri-state region, and operates under the direction of a local board, with a local focus.
