Join a team of compassionate, driven individuals and help offer a wide range of options for Allegheny families.DESCRIPTION OF WORKThese County Social Worker 2 positions serve as Health Enrollment Coordinators for Allegheny DHS, OCYF, under the manager of placement stability and the supervisor of health enrollment. Responsibilities include providing intensive care management and ensuring agency responsiveness to youth and family needs. Work involves coordination with Pennsylvania local and state Offices of Medical Assistance Programs including physical and behavioral health managed care organizations under Health Choices and private insurers to facilitate health enrollment and continuity of care. Collaboration is also key for working in consultation with the county solicitor and paralegal staff to provide documentation and testimony to inform the health status for case management and permanency planning.This position also supports transition age youth through direct education and advocacy as well as facilitates health enrollment for youth served through Juvenile Probation. - Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.

- Two years of professional social work experience in a public or private social services agency and graduation from an approved graduate school of social work.
