Primary Purpose:Assists individuals who are homeless, at-risk for homelessness, or medically fragile patients by providing supportive services that increase long-term stability in the community. Assist homeless or at-risk for homelessness individuals by enrolling them in the Community Integration Services (CIS). Responds comprehensively to the needs of clients through ongoing case management.Essential Duties:
  • Provides outreach services to individuals who are homeless or at risk for homelessness.
  • Carries a caseload of 20-25 CIS clients.
  • Must maintain a minimum of two face to face encounters a month and two telephone calls a month to each assigned client.
  • Assists clients with finding, securing, and maintaining permanent housing, and helps clients to reintegrate into the community.
  • Provides crisis and emergency services, either by telephone or face-to-face, as needed.
  • Conducts intake, assessment for clients upon enrollment
  • Provides clients with coaching and referrals for health and social services.
  • Conducts intensive outreach in the community and at the homeless shelters as needed.
  • Assists clients in acquiring emergency supplies, including food, clothing, bedding, medicine and hygiene items.
  • Assists clients in applying for entitlements and obtaining identification. Where needed, takes clients through entitlement, medical, and mental health systems to connect them with services and benefits.
  • Assists clients with legal entanglements.
  • Makes appointments for clients and escorts them to such appointments as needed.
  • Provides counseling, consultation, and crisis intervention as needed. Encourages medication adherence as appropriate.
  • Assists with family and social relationships as appropriate. Where needed, teaches communication and social skills development.
  • Provides appropriate case management services for each client which include:
  • outreach / engagement / housing needs and home visits
  • preparing a plan, based on clients' needs and goals, for the provision of services
  • following up with clients and providers of service to determine outcome of linkages
  • providing psycho-social rehabilitation services (teaching hygiene and self-care, housing skills, budgeting, problem-solving, etc.)
  • Advocates on behalf of clients to ensure equity of treatment.
  • Actively participates with the Coordinated Entry process, including case conferencing, coordination of outreach services, and other ad hoc meetings as required.
  • Keeps records of all client cases, and is responsible for ensuring that the records are entered into the clinic computer data-base, and for safeguarding clients' confidentiality and other rights.
  • Works as part of a team with other staff for purposes of outreach, back-up, and shared service delivery.
  • Assists other staff in obtaining information on clients from other agencies.
  • Documents all face-to-face, collateral, and telephonic services provided in AthenaOne.
  • Enroll CIS clients into the HMIS System and complete a VI-SPDAT
  • Advocates for clients to ensure equity of treatment.
  • Works with other KPHC staff and external partners in providing a coordinated system of care to clients.
Regular and reliable on-site attendance is required.Other Duties:· Adheres to Department-Wide and Program-Specific Job Expectations.· Performs other related duties as assigned.
  • Observes safety and security procedures; determines appropriate actions beyond guidelines; reports potentially unsafe conditions; uses equipment and materials properly.
Minimum Qualification Requirements:Skills/Knowledge:· Knowledge of housing issues and rental housing market in Hawaii.· Training in substance abuse / dual diagnosis.· Ability to relate effectively to difficult clients, including mentally ill, chemically dependent, and other homeless individuals.· Ability to operate a vehicle with valid State of Hawaii driver's license.· Must provide proof of automobile insurance and clean drivers abstract.· Ability to work independently in an effective manner.· Strong communication and interpersonal skills.· Must be able to work flexible hours to ensure responsibilities are met.· Ability to work as a team player.Education:· High school diploma or GED equivalent required.Experience:· Two years of providing direct services to homeless persons required. ouisng H· Knowledge of community resources and the Oahu service systems.Kalihi-Palama Health Center is an equal opportunity employer that is committed to diversity, and values the ways in which we are different.
