ShareLocated in Yuba City, CASalary: $22/hrCase Manager IFull Time/ Long Term $22/hrPosition SummaryCase Manager I (CMII) provides case management services to domestic violence and sexual assault survivors and their children (Shelter residents and non-residents). Case management services are built on the whole person care approach and aim to empower victims and survivors and assist them in regaining control of their lives. Community Support (CS) services include access to housing navigation, tenancy, and sustainability services; CMI also coordinates care with Case Managers II to access housing, medical, social, education, and other services. Case Management is on behalf of the Program participant and provides management of the case in collaboration with the client.Essential Duties and ResponsibilitiesThe essential functions include, but are not limited to, the following:
  • Development of an individualized, integrated care plan, including a housing plan.
  • Referral and related activities to help the Program participant obtain needed services.
  • Conduct regular case plans and update them as needed for changes in need for services. • Documentation of services in case plan.
  • Conducts a needs assessment of Program participant/member to determine eligibility and appropriate services and needs
  • Assist with TAR submission for CS members/clients.
  • Support the Program participants in their progress/regression and keep them focused on meeting their goals.
  • Provide Program participant transportation.
  • Provide support and crisis intervention to Program Participants.
  • Provide housing, legal, medical, financial, and employee advocacy to Program participants.
  • Make referrals for services to outside agencies.
  • Assist Program Participants in completing housing applications, including obtaining all documents necessary for a successful application, in interviews, and the grievance process should applications be denied.
  • Facilitate Program Participant moves once housing has been secured, including coordination, securing funds, assisting in moving property to the new residence, keeping accurate financial details concerning the cost of relocation, and completing the necessary documentation.
  • Assist Program Participant in obtaining a telephone or cell phone, utilities, driver''s license, SSI card, medical, and other pertinent needs.
  • Assist Program Participant with completing any required process for the housing provider''s certification/recertification process.
  • Develop and maintain relationships with the relevant property managers, leasing agents, and/or support services staff at the sites where we refer Program Participants for housing.
  • Provide professional, accessible, and culturally relevant support, advocacy, information, and referrals to Program participants.
  • Achieve a broad understanding of the housing-related resources and services available in the community for Program Participants, particularly those with acute medical and behavioral health needs. This may include rental assistance programs, food programs, and health and behavioral health services.
  • Participate in regular Coordinated Entry team meetings.
  • Prepares and submits reports as requested by the Director of Domestic and Sexual Violence Services
  • Always maintain confidentiality and discretion.
  • Encourages resident independence.
  • Develop engaging delivery methods for Domestic Violence and Sexual Assault victims while understanding the sensitivity and trauma-informed approach necessary in serving people who have experienced Domestic Violence and their children.
  • Remain current on Domestic Violence and Sexual Assault best practices and the resources available for Shelter residents, and make sure to keep Residents well informed.
  • Build community relationships on behalf of the Casa de Esperanza.
  • Perform other duties as assigned
Minimum Qualifications (Knowledge, Skills, and Abilities)
  • High School Diploma or GED, and;
  • 1-2 years social service experience or;
  • Associate degree in social services or related field or;
  • Principles and practices of leadership, motivation, team building, and conflict resolution.
  • Principles of business writing, basic presentations, proper grammar, spelling, punctuation, and composition of participant and Agency documents.
  • Office procedures, business mathematics application, computer office equipment and procedures, and statistical recordkeeping methods.
  • Principles and procedures of record keeping, reporting, and filing systems.
  • Principles, practices, and knowledge of Federal, State, and local laws of residential programs.
  • CalAIM Community Support (CS) services.
  • Domestic Violence and Sexual Assault Advocate Certification or obtain shortly after hire.
  • Valid California Driver’s License
  • Ability to build supportive and respectful working relationships with staff and residents.
  • Proven ability to work independently and as a team, being an effective and collaborative leader.
  • Skilled at communicating with a team responsible for their work but sharing the responsibility for the Program’s overall success.
