- Work with a caseload of clients from the CTI project through the 3 phases of the project to provide housing stability and community linkage.
- Assist clients with referrals and access to specialty care and community resources as needed.
- Coordinate with the client’s treatment team and support system to develop recovery and plans for maintaining stable housing.
- Ensure continuity of care by assisting clients in connecting to external resources and services.
- Accompany clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
- Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate.
- Model effective communication within the team model for a multi-disciplinary approach to client care.
- Coordinate care with interdisciplinary staff, outside entities, and family members to ensure continuity of care for clients.
- Provide mentorship, coaching, and training to new staff and student interns.
- Support in engagement activities to enroll new clients into the CTI project.
- Complete documentation for progress notes, assessments, exits, and tracking for the CTI project and documentation within Carelogic including notes.
- IMCANS certified and complete IMCANs assessment with clients enrolled in the CTI project.
- Actively participate in team meetings and staffing related to CTI clients.
- Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
- Assist clients in identifying signs and symptoms of de-compensation. Work collaboratively with clients to identify coping skills to manage symptoms.
- Assist clients in learning and improving independent living skills, i.e., personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items.
- Educate and assist clients with applying for entitlements, i.e., Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and SNAP benefits.
- Perform other related duties and/or projects as assigned.
- Must have one of the following requirements:
- Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
- Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
- Any other qualification that indicate the staff is an MHP
- IMCANS Certified and proficient in assessment and treatment planning or ability to become IMCANs certified within 60 days of hire
- Valid Illinois Driver’s License with daily access to a vehicle. Must have $100,000/$300,000 auto liability insurance.
- At least 1 year of experience working with people with mental health and/or substance abuse diagnoses
- Experience in community-based services preferred
