We are excited to expand our team and are seeking a compassionate and skilled LPN to join our Diabetic Infusion Therapy Program. This is an excellent opportunity to make a meaningful impact on patient outcomes while working in a collaborative and supportive environment.Position Overview:The LPN will play a vital role in supporting the operations of our Diabetic Infusion Therapy Program. Under the supervision of the Nurse Practitioner Program Manager, the LPN will provide direct patient care, assist with treatment administration, and contribute to the overall success of the program.Responsibilities:
- Patient Care:
- Assist in the administration of diabetic infusion therapy under the direction of the Nurse Practitioner.
- Monitor patient vital signs, glucose levels, and other pertinent indicators during treatment sessions.
- Provide patient education on self-care techniques, medication management, and lifestyle modifications.
- Clinical Support:
- Prepare treatment rooms, equipment, and supplies for patient appointments.
- Assist with the maintenance of medical records, including documenting patient assessments, treatments, and progress notes.
- Collaborate with the interdisciplinary team to ensure continuity of care and effective communication.
- Program Assistance:
- Support the Nurse Practitioner Program Manager in program operations, including scheduling appointments, coordinating referrals, and managing patient inquiries.
- Participate in quality improvement initiatives to enhance program efficiency and effectiveness.
- Graduate of an accredited Practical Nursing program.
- Current and licensed in Arizona.
- CPR certification required; additional certifications in phlebotomy or IV therapy preferred.
- Minimum of 1 year of experience in a clinical setting, preferably in diabetes care or related field.
- Strong organizational skills and attention to detail.
