- Doctorate in Counseling OR
- Clinical or Counseling Psychology OR
- Marriage and Family Therapy OR
- Behavioral Health or Mental Health
- AND Licensed Professional Counselor (LPC) OR
- Licensed Independent Marriage and Family Therapist (LMFT) OR
- Independent Licensed Clinical Mental Health Counselor (LCMHC)
- AND 2 years of clinical experience and/or experience in Counselor Education
- Master’s degree in the above areas
- AND 5 years of work experience as an independent licensed counselor in the above areas
- Your unofficial transcripts for any applicable conferred graduate degrees
- Any applicable licenses/certifications
