- Facilitate a daily communication forum for all team members to collaborate, plan and take action.
- Manage all referrals for the Community Treatment Team to include tracking, responding to the funder, maintaining active rosters and wait lists.
- Ensure all required clinical assessments are scheduled and completed within 30 days of admission and ensure all service activities provide a combination of in-person and virtual support modalities to meet the comfort of all individuals served.
- Develop and participate in the after-hours on-call rotation to promote community access and stabilization for individuals served.
- Submit billing and program expenditure documentation accurately and within required timelines.
- Master’s Degree, with LMHC, LICSW, BCBA or higher, required.
- Minimum of five years direct experience with individuals with Autism Spectrum Disorder and Significant Mental Illness, required.
- Direct experience implementing evidence-based practices, required.
- Valid driver’s license, acceptable driving record and reliable vehicle, required.
