- Conduct comprehensive assessments to evaluate patients’ functional status, needs, and goals related to their prosthetic or orthotic needs.
- Work closely with physicians, prosthetists, orthotists, and other healthcare professionals to determine the best care plan.
- Develop and implement personalized care plans that include device fitting, physical therapy, and follow-up care.
- Manage the scheduling of fittings, adjustments, and therapy sessions.
- Ensure that the care plan is executed efficiently and adjusted as necessary based on patient progress and feedback.
- Advocate for patients in obtaining necessary prosthetic and orthotic devices and services.
- Manage documentation and facilitate insurance claims and approvals to ensure that patients receive the benefits they are entitled to.
- Identify and connect patients with additional resources such as support groups, vocational training, and psychological counseling.
- Educate patients and families about device use, care, and maintenance.
- Provide emotional and psychological support to patients adjusting to new prosthetic or orthotic devices.
- Offer ongoing education tailored to changes in patients' conditions or advancements in prosthetic and orthotic technology.
- Maintain compliance with healthcare regulations and standards relevant to prosthetics and orthotics.
- Participate in audits and quality improvement initiatives to enhance service delivery and patient outcomes.
- Maintain comprehensive and accurate patient records.
- Document all case management activities and patient interactions.
- Prepare reports for healthcare providers and insurers as needed.
- Bachelor’s degree in Nursing, Healthcare Administration, Physical Therapy, or related field; Master’s degree preferred.
- Current professional healthcare licensure as applicable (e.g., RN, PT).
- Certification in case management (e.g., Certified Case Manager) is an asset.
- Experience in orthopedics, prosthetics, or orthotics is highly desirable.
- In-depth knowledge of prosthetic and orthotic devices and care needs.
- Strong organizational and communication skills to effectively coordinate with a multidisciplinary team.
- Compassionate patient care with a strong focus on empathy and patient advocacy.
- Proficiency in medical software and case management systems.
