Support improvements in health outcomes through coordinating care, educating patients, and building trust between patients and medical providers by enhancing communication and continuity of care. Works with patients who are living with HIV by linking them to Ryan White services and assisting them as needed once linked. Links patients at risk for HIV to supportive and preventative services. Provides community outreach, to people living with HIV and those at risk for contracting HIV. Provides health education and leads basic workforce development training and presentations. Works collaboratively and effectively within the Ryan White (RW) and PrEP team.Required Knowledge, Skills, and Abilities
  • LGBTQA+, HIV, STI, PReP, and family planning advocate
  • Strong interpersonal and communication skills
  • Strong commitment to the community and its members
  • Ability to work with community members from diverse backgrounds
  • Ability to motivate and work with individuals to promote participation in the project
  • Ability to work independently and within a team
  • Willingness to participate in a comprehensive, competency-based ambassador training program
  • Willingness and ability to travel by car to and work in community spaces
  • Ability to walk up two flights of stairs and carry up to 10 pounds
Required Education and/or Experience
  • Bachelors' Degree
Preferred Qualifications
  • Masters' Degree in Social Work (RSW, MSW, LCSW), Health Science Education, Public Health or closely related social/human service field.
