- Provides person-centered case management utilizing identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being
- Provide crisis stabilization services to individuals experiencing a mental health crisis while at the Sunrise House
- Conducts trauma-informed risk assessments as needed with the guidance of a Qualified Mental Health Professional to address safety concerns
- Collaborates with caregivers, family members, physicians and other treatment providers regularly until the crisis episode is complete
- With the oversight of a clinician, assists in coordinating access to appropriate level of care following assessment
- Utilizes deescalating skills in intense situations
- Supports consumers in achieving treatment goals, coordinate with collateral contacts, medication compliance, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed
- Create and initiate plans to assist clients with scheduling follow up mental health appointments
- Work towards successful reintegration of consumers back into their home, community, school and outpatient/community mental health services
- Schedule may include some night and weekend shifts base on community crisis needs and in coordination with direct manager/director
- Complete all required documentation, training and competency reviews in a timely and accurate manner as defined by the agency.

- Bachelor’s degree or equivalent years of work plus experience in a related field.
- Must be 21 years of age.
- Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
- Three to five years’ professional experience in mental health services, crisis response, or a related field.
- Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.
- Employee must complete CPST Case Management on-line training, KHS on-line training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
- Within 60 days of employment, the employee must have completed Adult Case Management training, CPR and First Aid Training.
