Crisis Stabilization Services at Horizons Mental Health Center (HMHC) offer 24/7, short-term services, to adult and youth experiencing a mental health crisis with the mission to reduce inpatient hospitalization admissions, reducing the amount of time with law enforcement, and engaging individuals with outpatient treatment to support their needs.Provides intensive, goal directed services to support individuals who are in a mental health crisis which require crisis stabilization services. The team works closely with all Horizons clinical and community partners to provide services at a higher level of intensity or frequency with the goal of community tenure by the consumer.HMHC is a Kansas, not-for-profit organization, committed to providing comprehensive mental health services to individuals in Kingman, Pratt, Reno, Harper, and Barber Counties.   • Provides person-centered case management utilizing identified and accepted methods to remediate symptoms of mental illness and/or improve emotional and functional well-being
  • Provide crisis stabilization services to individuals experiencing a mental health crisis while at the Sunrise House
  • Conducts trauma-informed risk assessments as needed with the guidance of a Qualified Mental Health Professional to address safety concerns
  • Collaborates with caregivers, family members, physicians and other treatment providers regularly until the crisis episode is complete
  • With the oversight of a clinician, assists in coordinating access to appropriate level of care following assessment
  • Utilizes deescalating skills in intense situations
  • Supports consumers in achieving treatment goals, coordinate with collateral contacts, medication compliance, crisis management, prompt client to complete daily living activities, monitor client one to one for safety or as needed
  • Create and initiate plans to assist clients with scheduling follow up mental health appointments
  • Work towards successful reintegration of consumers back into their home, community, school and outpatient/community mental health services
  • Schedule may include some night and weekend shifts base on community crisis needs and in coordination with direct manager/director
  • Complete all required documentation, training and competency reviews in a timely and accurate manner as defined by the agency.
  • Other activities as assigned.
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:

MINIMUM EDUCATION & EXPERIENCE REQUIRED:
  • Bachelor’s degree or equivalent years of work plus experience in a related field.
  • Must be 21 years of age.
PREFERRED EDUCATION AND EXPERIENCE:
  • Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education (with specialized training in human development and family dynamics).
  • Three to five years’ professional experience in mental health services, crisis response, or a related field.
  • Two years or more of experience working with educating and teaching clients about self-care, improving daily living activities.
LICENSURE OR REGISTRATION REQUIRED:
  • Employee must complete CPST Case Management on-line training, KHS on-line training, pass KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens.
  • Within 60 days of employment, the employee must have completed Adult Case Management training, CPR and First Aid Training.
  • Valid driver’s license, insurance, and a dependable vehicle
