Job Description:Up to $5,000 Sign-On bonus based on experience.The Clinical Pharmacist Specialist is under the supervision of the Regional Clinical Coordinator and the Regional Directory of Pharmacy will work directly with the health care team and patients to provide safe and effective pharmacy services in addition to the dispensing of medications. Provides consultative services to medical, nursing and pharmacy staff. Ensures appropriate drug therapy, implements and monitors clinical protocols and guidelines, and enhances the delivery of evidence-based medicine. Other responsibilities include education, representation on medical center committees, and collaboration on clinical initiatives. Documents clinical activities and outcomes. Provides prescribed medications and compounds according to professional standards and practice. Performs at an advanced skills level. Prioritizes work demands. Participates in various professional education activities, which includes working with students from the local Colleges of Pharmacy and, Pharmacy Practice Residents. Assists with development and mentoring of clinical staff pharmacist. Completes established competencies for the position within designated introductory period. Other related duties as assigned.MINIMUM EDUCATION: Graduate of accredited College of PharmacyPREFERRED EDUCATION : Doctor of Pharmacy DegreeMINIMUM EXPERIENCE : 2 years hospital clinical experience in Critical Care or 1 year hospital clinical experience in Critical Care and completion of an accredited Critical Care residency program.PREFERRED EXPERIENCE: 5 years hospital clinical experience in Critical CareREQUIRED CERTIFICATIONS/LICENSURE: Possession of current Texas State Board of Pharmacy License or eligible for reciprocity.PREFERRED CERTIFICATIONS/LICENSURE: Board Certified Pharmacotherapy Specialist-BCPS#LI-JH5Tenet complies with federal, state, and/or local laws regarding mandatory vaccination of its workforce. If you receive an exemption from the vaccination requirement, you will be required to submit to regular testing in accordance with the law.Responsibilities:This individual's responsibilities include (but are not limited to): All general responsibilities of a Staff Pharmacist. Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals. Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions. Deliver medication therapy management within specialized patient care areas and ensure pharmaceutical care programs are appropriately integrated throughout the institution. Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to medication therapy management. Conduct Medication Use Evaluations (MUEs) and assist in making necessary changes to pharmacotherapy practice as needed based on MUE findings. Review and analyze clinical data to make informed decisions for patients with complex clinical conditions. Develop and oversee drug utilization, distribution, and control in order to provide quality and cost effective care. Facilitate experiential education by developing training and competency for students, residents, and/or allied health professionals. Provide at least one in-service program to pharmacy staff annually and as needed. Provide at least one in-service program to nursing or physician staff annually and as needed. Consistently participate in one or more interdisciplinary committee(s).Qualifications:Education:Required: Pharm.
D. or BS in PharmacyExperience:Required: 4 years of pharmacy practice experience in area of specialty or; PGY-1 pharmacy practice residency plus 2 years of pharmacy practice experience in area of specialty or; PGY-2 pharmacy practice residencyCertifications:Required: Pharmacy license. AHA BLS, ACLS, PALS required if direct patient care responsibilities. Board Certification in specialty area (e.g., Cardiology, Critical Care, Infectious Diseases, Oncology, Pediatrics, etc.).Physical Demands:Lift/position up to 50 lbs. Push/pull up to 50 lbs. of force. Frequent standing and walking. Frequent reaching, stooping, and bending.
