- Receiving orders and care plans from a virtual care team and preparing daily patient assessments for patients during the acute phases of their illnesses
- Collaborating with virtual nurses and hospitalists to identify needs and interventions for hospital at home patients and providing ongoing support for advanced care planning
- Participating in home visits and transition of care management
- Administering nursing interventions, such as vascular access care, infusion, and wound care
- Providing coaching and assistance to patients regarding treatment decisions as well as other health education support
- Making evidence-based clinical decisions that align with medical necessity guidelines, practice standards, industry standards, best practices, and contractual requirements
- Complying with all requirements related to scope of practice, facility policies, and legal directives
- Leading clinical operations and medical management across the entire continuum of care (assessing, planning, implementing, coordinating, monitoring, and evaluating)
- Receiving information from patients about specific symptoms and questions
- Maintaining patient engagement by establishing rapport, demonstrating empathy, and building a trusting relationship
- Active Massachusetts state Registered Nurse (RN) license
- Minimum 2 years experience in an acute care, in-patient, hospital setting (1-3 years of recent experience)
- Ability to place a peripheral line with experience maintaining and accessing midlines and PICCs
- Current CPR for Healthcare Providers Certification from the American Heart Association
- Ability to travel to multiple patients' homes during your shift and to work independently
- Strong clinical and assessment skills
- Hepatitis B and MMR records, and current influenza vaccination
