  • Ability to perform resident assessments
  • Create and maintain individualized service plans
  • Full knowledge of DOH regulations
  • Scheduling of resident appointments including but not limited to transportation
  • Assure compliance with accordance to the state regulations
  • Schedule and conduct family meetings
  • Computer literate
