  • Provides education and support for facility social services team members. This includes education on discharge planning, discharge processes, social services documentation, RAI process completion, trauma informed care, and overall psychosocial needs for the residents.
  • Provides education and support for facility activity team members. This includes education on the RAI process and overall activity programs.
  • Conducts calls at least quarterly for all activity and social team members.
  • Assist facilities with educational requirements related to behavioral health needs. This includes trauma informed care, cultural competence, substance use and suicidal prevention.
  • Provides dementia care training for interdisciplinary team for facilities that have a dementia care unit.
  • Works with clinical education team to ensure that nursing departments are also educated on dementia care and behavioral health needs.
  • Serves as resident rights champion in the facilities.
  • Monitors and tracks abuse allegations and identifies corporate initiatives to minimize occurrences.
  • Coordinates quarterly customer satisfaction surveys for skilled facilities.
  • Develops and implements a Veteran’s Care Program for the company.
  • Audits facility compliance with regulatory requirements related to social services and activities.
  • Audits facility compliance with behavioral health and dementia care programs.
  • Audits Veteran Program at contracted facilities.
  • Serves as subject matter expert for volunteer programs in the facility.
  • Other Duties as assigned
JOB REQUIREMENTS:
  • BS/BA degree and a minimum of 4 years’ supervisory experience in a nursing facility required.
  • Must be certified in North Carolina as a social worker.
  • Must be willing to work with various facilities, department directors, patients, visitors, family members and community leaders.
  • Must have the ability to plan, organize and follow-up on work assignments.
  • Must have a sound knowledge of personnel administration and must be able to supervise all employees.
  • Must have a sound knowledge of local, Federal and State regulations, Medicare and Medicaid regulations, nursing and medical practices and procedures, and any other laws pertaining to Nursing Facility operation.
  • Must have the ability to make independent decisions and work well under pressure.
  • Must have knowledge of economic conditions and ability to adjust cost of operation to meet community ability to pay for services.
  • Must have the ability to keep abreast of changes in the Nursing Home industry.
  • Must be able to interpret financial statements and prepare monthly reports for Long Term Care Management Services.
  • Must have the ability to promote public relations.
