Resident Care CoordinatorAmerican GrovesGilbert, AZResponsibilities:
  • Ensure the quality of care of all residents
  • Observes resident's health status, take actions to address concerns with scope of practice, record changes, and notify Health Director
  • Willingness to train caregivers in tasks required for delivering quality care to each resident
  • Conducts coordination of care plan meetings with residents and/or their family members
  • Schedules the service plan dates according to state regulation guidelines
  • Assure home health, hospice and ancillary care providers communicate their visits and provide necessary documentation of their visit
  • General record keeping and filing as needed
  • Must have the availability to cover open shifts (i.e., when call offs occur or staffing shortages) when he/she is unable to cover open shifts by other care team members
  • Supports Health Director and Executive Director in all aspects of onboarding and training of new care team members
  • Collaborate with leadership team to create and maintain care schedule that accommodates requested days off, vacations and sick time while maintaining appropriate staffing levels according to resident care needs
  • Contribute to team effort by being flexible in work assignments - filling in for Med Aide and Caregivers as needed to cover shifts or assigned by health services leadership to stay apprised of resident care concerns and to educate, train and guide health services team members
  • Build positive family and POA relationships to ensure customer's satisfaction
Required Qualifications:
  • Arizona Caregiver Certification
  • CPR/1st Aid Card
  • Food Handler's Card
  • Passion to serve seniors
  • Ability to relate well to residents and their families
  • Demonstrates good judgement, problem solving and decision-making skills
  • Self-starter, creative, organized, and professional
  • High School diploma or GED equivalent
  • Must be at least 18 years of age
