SummaryPOSITION SUMMARYPosition Summary:This position is responsible for delivery of direct patient care through the nursing process of assessment, planning, implementation and evaluation; is responsible for identifying and coordinating patient/family needs and for providing professional nursing care on an intermittent basis in the patient's place of residence.
