- LMSW or LCSW in the state of Connecticut.
- Minimum of 1 year of clinical experience; Hospice, Home Care, Acute Care, and/or Bereavement Counseling preferred.
- Have reliable transportation and be willing to commute within a designated territory to provide the best in-home patient care.
- Demonstrates good verbal and written communication and organization skills.
- Provide Clinical Social Work to Hospice and Home Care populations.
- Assesses the psychosocial status of patients and families/caregivers related to the patient's medical condition and/or terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
- Provides an assessment of the patient's identified residence and assistance when this is not safe, and another plan is required.
- Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, neglect or abuse and plan intervention based on evaluation findings.
- Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
- Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record-keeping system. Submits all clinical documentation, time records and other information in the required time frame.
- Other duties as delegated by the Clinical Supervisor.
