The PT will develop and update, as often as indicated, an individualized plan of care including, but not limited to the patient's functional levels, goals, interventions, and rehabilitation potential.Education
  • Graduate of an approved school of Physical Therapy (PT).
Licenses/Certifications
  • Current state Physical Therapy license.
  • Must meet initial and ongoing educational requirements as stipulated by the state to maintain an active professional license.
  • Current CPR card, valid driver's license, reliable transportation, automobile insurance.
