- Supportive and collaborative work environmentResponsibilities:- Provide direct patient care in the comfort of the client's home
- Conduct initial and ongoing assessments of the client's health status
- Develop and implement personalized care plans based on assessment findings
- Administer medications and treatments as prescribed by the physician
- Monitor and record changes in the client's health status
- Educate clients and their families on proper health maintenance and disease prevention strategies
- Collaborate with other healthcare professionals to ensure the best possible outcomes for clients
- Communicate and coordinate with the client's physician, family, and interdisciplinary team members
- Adhere to all company policies, procedures, and standards of care
- Maintain accurate and timely documentation of all care providedRequirements:- Active and unrestricted RN or LPN license in the state of Florida
- Minimum of 1-year nursing experience, preferably in a home health or hospice setting
- Current CPR certification
- Strong clinical skills and knowledge of nursing best practices
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver's license
- Passion for providing compassionate and patient-centered care
