About our Team:We employ a field-based, home-based care model and are committed to meeting members where they are--in their homes, in their community, and in our Hubs. You will go above and beyond to connect with Cityblock members in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.About the Role:In this role, you will be a part of a cohort of staff providing direct services to members, who face economic and social challenges in addition to having mental and physical health needs, through our innovative care model.Reporting to the Hub Operations Manager, (HOM) the Senior Hub Operations Associate is a key member of the Cityblock care team and plays an integral role in defining and delivering a state-of-the-art experience to Cityblock members. Senior HOAs act not only as the essential point of contact in our clinical hubs, but also play a vital role in the overall flow of the hub. Duties include managing vendor relationships and facilities management. Other duties include coordinating provider support, community-based programming in the hub and tracking member activity using both Epic and Commons, Cityblock's proprietary care facilitation system. The Senior HOA ensures that, regardless of the circumstances, members are treated with respect and dignity, and always feel at home and the hub has what it needs to operate optimally for member experience.You will:
  • Be the onsite resource at the Highpoint Cityblock Hub, a clinical environment for our care team members. Working alongside the care teams you will support member needs by collaborating with external partners to address member's social determinants of health.
  • Identify operational gaps with a solution in mind
  • Manage Commons driven reports for HOM
  • Orchestrate an excellent member experience at the Hub by ensuring routine day to day operations in partnership with the HOM.
  • Provider and hub team onboarding - SHOA
  • Manage (review and distribute) incoming hub faxes
  • Additional job related functions, as necessary
  • Address operational issues as they arise, and collaborate with the HOM
  • Oversee assigned team members, fostering an environment of open communication
  • Oversee and ensure effective use of technology and applications, including the EHR, Commons, and other tools used to support operations
  • Collaborate with HOM on staff scheduling needs.
  • Comply with environmental requirements, including equipment maintenance- SHOA
  • Provide coverage for the Medical Assistants as needed to maintain steady operations - COA
  • Assist the HOM with monitoring real-time member experience feedback.
  • Assist the HOM with ad hoc requests and tasks as needed.
Requirements for the Role:
  • Bachelor's degree preferred
  • Must have a minimum of 3-5 years of operational support work
  • Experience in the medical
  • Able to thrive in a fast paced environment and process and policies may change frequently
