The Social Worker assesses the psychosocial needs of the assigned patients/families coordinating a plan of care that meets the unique needs of the patient and his/her family. The Social Worker collaborates and participates in the disciplinary planning process and coordinates pre/post-hospital care, and effective use of community resources in order to integrate care, improve quality of care, and decrease cost and utilization.Position RequirementsGraduation from an accredited college or university, with a master's degree in Social Work, Sociology or related field. Minimum three to five years experience in clinical setting and supervisory experience required.
